3:00 PM to 5:30 PM Check-In/Registration @ Holiday Inn Express
6:00 PM Dinner and Opening Session

1. Number/specific nights you need a reservation – Thursday? Friday? Saturday (TSCA

2. Indicate if you want a room by yourself. If you want a room by yourself, you will be
responsible for 1/2 the room cost ($38 plus room tax, and any incidentals) and will be
expected to present a check made out to the Tennessee Counseling Association for the amount
you owe, to Treasurer John Mitchell prior to your departure. (TCA does not pay for
friends/family who accompany you to Cookeville but who are not eligible participants for
LDC).
3. Name of preferred roommate ________________________.
4. For guests, the hotel provides a complimentary “hot breakfast” each morning.
5. Chapter position/Division position/Governing Council position with name (i.e., CCA
Membership Chair, First & Last Name).
6. Pontoon boat ride Friday afternoon **optional**. Ice/water/soft beverages provided. You
may prefer to use that afternoon for your own personal time: maybe a dip in the hotel
swimming pool; shopping at some cool shops in the Broad/Cedar Streets and Broad
Street/Square areas; maybe a good book and a quick nap.
(You must RSVP for boat ride, no later than June 1st!)

7. TCA will provide lunch and dinner on Friday.
ALL CHAPTERS/DIVISIONS must bring President, President-Elect, Membership Chair and an Emerging Leader (preferably 4 different people for leadership development!). TCA will pay for these 4 individuals. If you are traveling from the same areas, please carpool if possible. This will allow the organization to keep mileage reimbursement costs at a minimum.
NOTE TO ALL TCA MEMBERS!!
ANY TCA member is welcome and invited to attend and participate in LDC. However, if you are not an officer or you are not a new emerging leader or membership chair of a TCA chapter or division, you will be responsible for all of your own expenses.
